This company appears to be a wholesaler that sells to Dentists and big stores like CVS. It seems like their sample form is for industry folks like Dentists to try these and buy them in bulk. The sample offer has been around since February, but I so far I haven't heard of anyone receiving their samples. It's worth a shot, but don't be disappointed if they don't come.
